Macmillan Cancer Support have created an informative booklet to give guidance on your end of life choices. You can download the booklet as a PDF by clicking on the link above. However, remember you do not need to have been diagnosed with a terminal illness before you prepare your end of life choices.
Here are some simple things to help you think about preparing for your own death:
Where would you prefer to die? For example – in hospital, at home, or in a hospice?
Ensure you tell you family where your Will is stored, and how to access your financial records.
Would you prefer to be buried or cremated? Also, do you have a preference on where?
How would you like to reflect your personality at your funeral (if you would like one)?

UHNM Palliative Care
Locally, the UHNM (Royal Stoke) Specialist Palliative Care team aims to improve the quality of life for patients with life limiting illness. They see patients at any point during their illness, not just at the end of their life. They are able to support patients who are still receiving active treatment, and early referrals are encouraged.
Referrals may be made by healthcare professionals working at UHNM (e.g. Dr’s, Nurses, Allied Healthcare Professionals) or other organisations (e.g. hospice).
